This gives patients the opportuntiy to form strong bonds with family medicine providers while having access to other specialists. Patients report forming strong relationships with not only their providers, but with team medical assistants, nurses, and care coordinators. Together, the team increases communication between patient and provider, while giving each patient a stake in their own health care. Streamlined referrals within PCC and with our network partners also help make sure patient needs receive the correct follow-up. 

Clinical Outcomes

PCC monitors internal quality for the entire organization on a regular basis. We aim to meet Healthy People 2020 goals, such as increasing access to health care. PCC wants patients to lead healthier lives too. To do this, we track trends in health outcomes to determine how our care affects patient health. Examples of clinical outcomes that we track include:

  • Depression screening and follow-up care
  • Birth weight
  • Glucose (sugar) control for people with diabetes
  • Hypertension (high blood pressure)
  • Immunizations
  • Dental decay
